Business Applications
Below are official applications for your annual business license for those doing business within the city limits of Belle. These forms are made available to you to print. After you complete any of the application forms, please include required payment. They are to be delivered to or mailed to:

Dog License Permit Applications
Belle City Ordinance (refer to ordinance, Section 205.020) requires all dogs be registered. You must provide proof of current rabies shot. An annual $7.50 registration fee is charged for each dog (refer to ordinance, Section 205.040). Special rules pertain to pit bull animals and you need to contact the City Clerk at 573-859-3513.
ATV, Scooter or Other Motorized Means of Transportation
Belle City Ordinance, Section 340.220, requires you to obtain a valid city permit to operate ATVs, scooters and other motorized vehicles on the city streets. You must show a valid driver’s license and proof of liability insurance. An annual fee of $15.00 is charged for each ATV, scooter, or motorized vehicle operated within the city limits.
Water/Sewer Application
All residents who own or rent a home or property must complete an application for water/sewer service. If the person is renting a home, a $50 deposit is required. All applications must contain a current mailing address. If you do not have a mailing address, you are required to contact the Belle Post Office and apply for a post office box.
Water Shut Off Application
Any resident who moves from his/her physical address must submit an application to the Belle City Clerk to have the water/sewer service shut off (disconnected). You may download the following application. Please bring the application to City Hall or fax or email it to the City Clerk. This form must be filed with teh City Clerk before the water/sewer account can be taken out of your name.
Request for Police or Motor Vehicle Accident Report
You may request a copy of any police or motor vehicle accident report from the Belle Police Department. The fee required for producing the report is $10. You may download the following application and submit it with the required $10 fee to the Belle Police Department, P.O. Box 813, Belle, Mo., 65013.
